Request for Modification
Individuals seeking modifications to access Court Proceedings, programs, services, court records, or activities at the Wyoming Supreme Court, Chancery Court, any District Court, or any Circuit Court are encouraged to submit a Request for Modification Form to the Statewide ADA Coordinator. While a written Request for Modification is preferred, individuals may also make the request by telephone to the Statewide ADA Coordinator. The Request for Modification Form is accessible through the link below, or can be obtained from various locations, including the Statewide ADA Coordinator, the Supreme Court Clerk’s Office, the Chancery Court Clerk’s Office, any District Court judicial assistant, and any Circuit Court Clerk’s Office. Additionally, individuals can request the form in large print or other accessible formats as needed.
A Request for Modification may be submitted by any lawyer, party, witness, juror, or other individual interested in attending any Court Proceeding, program, activity or service or another person on behalf of such interested person.
The Request for Modification should be submitted to the Statewide ADA Coordinator with as much advance notice as possible.
